The Addiction Family Support Navigator Program will work with service providers to enhance linkages to existing services and to connect families to supportive services.
The Family Navigator is well-versed in the following areas in order to address family needs of patients with co-occurring and substance use disorders:
- Substance Use Treatment Facilities
- Inpatient
- Residential Treatment
- Outpatient Services
- Housing Services
- Mental health services
- Vocational and educational services
- Health care
- Health Home coordination
- Risk assessment
- Prevention services
- Supportive counseling services
The program provides additional family support amenities for the community inclusive of CRAFT Group, Grief Recovery Method groups, and connection to peer advocate support services.
When barriers in the receiving of services occur, the Family Navigation Program will provide advocacy for the family and recipients of services. The expectation of the provision of family navigation services encompasses the securing, utilization, and completion of co-occurring and substance use services that may improve and promote increased family and community wellness.
